Forms to request library materials and/or services are readily available in the Library and electronically. Additionally, the following forms may be submitted electronically to the Library by clicking on the form below.

• CBks Form 899-R-E (Request for Copyright Permission) (CBOnline Access Only)
• CBks Form 658-R-E (Request for Information Resource) for purchases with departmental funds (CBOnline Access Only)
• CBks Form 968-R (Recommendations for Library Material)
